Payment for Services

Patients are requested to pay at the time services are rendered unless prearranged by our financial staff. We file your insurance and work with you within the parameters of your plan to help you receive the maximum benefit from your coverage. Remember insurance plans vary in coverage and you are responsible for knowing details of your insurance plan.

We know that as you prepare for your visit these questions can seem overwhelming:
  • Will my procedure or treatment be covered?
  • What is my deductible? How much do I have to pay?
  • Do my Medicare benefits provide coverage for this treatment?
  • Do I need a referral?
To receive the best treatment covered under a patient’s health plan, the patients, the physicians and the health plan must work together as a team.

We recommend that you:
  • Determine your insurance coverage.
  • Receive approval from your insurer before undergoing or starting a treatment plan.
  • Talk to your physician about treatment options.
  • Check with our financial services department for insurance coverage issues.
  • Talk to your health care plan member services department.
  • In all cases, the patient is responsible for understanding their insurance coverage and is ultimately responsible for their bill for services rendered.
Forms of Acceptable Payment:

Urology Center of Columbus accepts cash, checks, VISA, MasterCard and Discover. In the event that a check is returned as non-payable, Urology Center of Columbus will charge a service fee. We now offer Care-Credit. With this plan, you can begin your treatment and care immediately and pay over time with low monthly payments. Please go to www.carecredit.com. If you do not have insurance, we will require payment at the time of service. You should contact our office at (706) 324-7700 to discuss payment arrangements if necessary.

Copays/coinsurance/deductibles:

All co-pays/coinsurance/deductibles required by your insurance plan are collected at the time of service. Patients receiving urodynamic services should be aware that although these services are diagnostic in nature, they may be considered surgical by your insurance company and therefore may require a separate co-pay or coinsurance.

Referrals/pre-certifications/prior authorization:

Should your visit require a referral from your primary physician, you should ensure that the referral (document) has reached our office prior to your visit. Your visit will be rescheduled if Urology Center of Columbus does not receive your referral.

If you choose to be seen without the appropriate referral in hand, you agree to be responsible for the charges should they not be covered by your insurance.

Disputes:

If for any reason you dispute coverage or payments made by your insurance company, it is your responsibility to contact the insurance company and to resolve the matter based on your insurance company’s arbitration or resolution process. We will provide documentation (providing your signature of authorization is on file) to assist in the dispute resolution process. During this time you will be asked to pay in full the balance or schedule payment arrangements by contacting the office at (706) 324-7700.

Collection

If all reasonable attempts of collecting payment fail, Urology Center of Columbus reserves the right to use the service of collection agencies or court judgments.
