New Patient Registration

New patients making their first visit to Urology Center of Columbus are requested to arrive 15 minutes before scheduled appointments for the purpose of registration. Make sure to bring your driver’s license, insurance cards, your medications/herbals, and your co-pay required by your insurance as well as referral from your primary physician if required.
